Q. How do I obtain a copy of my Ontario Student Transcript?
A. If you are still in school or left our school within the past five years:
please fully complete a "Guidance Document Request Form" and send/deliver it to the Guidance Secretary. Please allow at least 24 hours for processing--if you need it sooner, please call ahead. There is no charge for students currently registered at CI. For all others, the fee is $5.00 for the first transcript and $1.00 for each additional one.

Q. How do I make an appointment with a Guidance Counsellor?
A. Complete a "Request for Appointment" (located on the filing cabinet just inside the door) and put it on the "spike". Appointments are normally processed on a daily basis and the slips are distributed to you via your home room teacher the next morning.

Q. How do I drop or change a class?
A. Make an appointment with a counsellor to discuss your intentions. Should a drop or change be in order, a form may need to be completed and signed before the drop or change takes place.

Q. How do I make an appointment with any of the support staff?
A. See the Guidance Secretary if you wish to make an appointment with the School Nurse, Youth Counsellor, or Ontario Works Case Manager. Please note that support staff are not in every day but do have a weekly schedule of visits to CI.

Q. How do I sign up for presentations put on by the Guidance Department?
A. You will find the "Guidance Presentations" sign-up binder next to the appointment slips in the Guidance Office. Locate the applicable page for the presentation, and print your name and that of your home room/Period 2 teacher in the appropriate columns. Make note of the date, time, and
location of the presentation in your agenda so you don't forget about it. You must sign up in advance in order to attend.

Q. How do I obtain a reference letter?
A. Firstly, you should ensure that the person whom you wish to provide you with the reference is willing to do so. Secondly, you should print the "Reference Request & Authorization" form. Complete the form neatly and in full, sign it and submit it to the appropriate individual. You should
allow at least a week for processing.

Q. Can I borrow items from the Guidance Office?
A. Just about anything is available for you to borrow. Before you take anything, however, you must sign it out. You will find the "Guidance Materials" sign-out binder on the lower left shelf of the "College Information" section in the Guidance Department. Please read the instructions on the opening page of the binder which will provide you with further direction.
